Positional vertigo: Exercises For treatment at home

Positional vertigo is vestibular disorder that affects millions of people throughout the world. Positional vertigo is also recognized as benign paroxysmal positional vertigo (BPPV). It is estimated that 9/10 older people suffer from BPPV. It is more common in women compared to the men. People suffering from BPPV experience vertigo, intense dizziness when they turn or move head quickly, bend over, roll over in bed or tip their head back. Other symptoms of this condition are imbalance, nausea and lightheadedness. BPPV goes away itself in a few weeks, but medications are available to improve the symptoms. Many people recover from BPPV with simple and easy exercises. Here are provided some of the at-home positional vertigo exercises.

A problem in the inner ear leads to positional vertigo. BPPV is happen when the pieces of calcium carbonate material layer in the utricle (a structure in the inner ear) break off and move to the semicircular canals (another part of the inner ear). These tiny calcium crystals (otoconia) are known as ear rocks. Because of infection or inflammation, the ear rocks are not able to move as they should. This sends wrong signals to the brain, which affects our balance and causes the vertigo. The feeling of vertigo last for a few seconds or up to a minute and can be mild or severe enough that you lose your balance while sitting up, standing or walking.

Exercises For Positional Vertigo

Three types of exercises: Epley maneuver, Semont maneuver and Brandt-Daroff exercise are available for positional vertigo. Epley maneuver is helpful in over 90% of cases; Semont maneuver is less common but 90% effective while the Brandt-Daroff exercise is time-consuming but effective for most people. These exercises help the ear rocks to move freely so that they do not cause vertigo. You must talk to your doctor before starting any exercise.

Epley maneuver

Epley maneuver is also known as particle repositioning or canalith repositioning maneuver. In the Epley maneuver, you lie down and move your head in into four different positions (about 30 seconds for every position). Moving your head in different directions can cause nausea or vomiting. If you feel so, then consume an anti-nausea medicine.

Steps to Perform
  • First of all, sit in upright position for 1 minute and turn your head to the left side at a 45 degree angle.
  • Lie on your back with your head turned and stay in the position for 5 minutes.
  • Now, turn your head to the right side at a 45 degree angle and stay in the position for about 5 minutes.
  • Then, turn your body onto your right side, keeping your face downwards.
  • Remain in the position for another 5 minutes and return to the starting position. Stay in the position for about 30 seconds and repeat the steps for 2 to 3 times.

Semont Maneuver
Semont Maneuver is also recognized as liberatory maneuver. In the Semont maneuver, you lie down and move your head firmly in several positions. The session continues up to 10 - 15 minutes.

Steps to Perform
  • Sit on a bed, keeping the legs hanging off the side and turn your head at 45 degrees angle horizontally in the direction of the unaffected ear.
  • Then, lie down on the affected side with your head turned and stay in the position for 3 minutes.
  • Now, return to the starting position and wait for about 20 to 30 seconds.
  • Turn your head at 45 degrees to other side and repeat the same process in the opposite direction.

Brandt-Daroff Exercise
Brandt-Daroff Exercise is usually selected when Epley or Semont maneuvers don’t work. In this Brandt-Daroff exercise, you have to shift from a sitting position to a lying position repetitively till the vertigo goes away.

Steps to Perform
  • Sit straight on the edge of the bed or table and turn your head at 45 degrees to the left side.
  • Lie down swiftly on your back to the right side and stay in the position for 20 to 30 seconds.
  • Move back to the original position and wait for 20 to 30 seconds.
  • Turn your head at 45 degrees to the right side and after that swiftly lie down on your back to the left side. Stay in the position for 20 to 30 seconds.
  • Return to the starting position again and wait 20–30 seconds
  • Perform it 5 times on each side.
